Providing Linkages for the Underserved to Services
Health P.L.U.S. is a grassroots initiative, created to address the intersectional health and social needs of underserved women in rural Alachua, Levy, and Putnam counties.
Community Health Workers (CHWs) will share information about resources, provide linkages to health and social services, and offer emotional support, including motivational text messages.
For those with a need for more specialized assistance, a CHW on-site can take a referral for the Health P.L.U.S. Navigator or they can contact us by calling or texting the Health P.L.U.S. help line – 352-514-0237.
Health P.L.U.S. was conceptualized with a central premise: when you meet women where they are, you can improve their health and overall well-being. This is the reasoning behind our unconventional outreach, as much of it takes place at food distribution sites, churches, libraries, and coalition meetings across the three counties.
